Following are the health plans which I currently accept.

If your plan is not listed, I will be unable to file a claim on your behalf. However, I can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

For those who are uninsured, a fee will be charged according to a sliding scale.

Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

All fees, copays, and deductibles are due at the time of service.

You are responsible for communicating directly with me if you are unable to keep an appointment. For all scheduled appointments not cancelled within 24 hours of appointed time, a missed appointment fee of $75 will be charged.
